Post-Project Evaluation Form
Please let us know how your funded project went! This form is mandatory for all funded projects. An individual or group will not be able to re-apply for funding until we have received a post-project for your previous application!
What we are looking for: Your post-project evaluation form should consist of a detailed reflection as to what worked and what did not in terms of your project’s success. Photos of the completed project are required. We use these to spread the word about all the amazing funded projects, and also to see what the final product of your project looks like. The detailed the better- we really appreciate your feedback and evaluations!
